E & O Insurance
In order to have E&O Insurance through Protective Life, you must be “contracted” with them first.
Please complete these steps:
Step 1:
You must fill out the
Accelerator Normal Agent Contract 6.1.06.pdf. application.
Then fax Contract Application to 412-747-7778.
Step 2:
After your fax is received, you will be notified by EBS. Please wait to hear from EBS before Continuing to Step 2.
The three following forms will be required to further process your E&O Coverage.
